Monitor has a statutory duty to publish a directory of foundation trusts on its website. This directory includes the members of each foundation trust’s board of directors and board of governors. Trusts are required to provide these details to Monitor as part of the Compliance Framework.
We have removed the director and governor updates from the monitoring sheet which foundation trusts are required to complete each quarter. We have replaced this element of the monitoring sheet with the forms on this webpage, enabling foundation trusts to update their details at any point during the year. Please submit any changes to your board of directors or governors by accessing these forms below:
Once you have completed the forms and pressed 'submit' you will see a page with text confirming that your changes have been submitted to Monitor - you will not receive a separate email to confirm that we have received your changes. Monitor’s Communications team will then manually update the directory on the website with the changes you have submitted. Please note this may take up to two weeks.
