Schedule 6 of an NHS foundation trust's terms of authorisation lists the information they have a legal or statutory obligation to supply. The latest version is available below.

Schedule 6 aims to streamline requests for information to NHS foundation trust which do not comply with the following principles:

  • only necessary information should be collected;
  • collection should be performed at least cost;
  • duplication should be avoided; and
  • information should be made available quickly and usefully.

The NHS Information Centre runs an approval process for requests for information. All requests for information, which NHS foundation trusts do not have a legal or statutory obligation to complete, will require Monitor approval through this process before becoming mandatory.

Approved requests will then be added to Schedule 6 of the NHS foundation trusts' Terms of Authorisation and reviewed periodically.
