The latest version of Schedule 6 is available to download above.
Schedule 6 was created as a result of work between Monitor, the Department of Health (DH) and the Healthcare Commission to streamline requests for information which do not comply with the following principles:
Building on the work to streamline existing reporting, Monitor and the DH developed an approval process for future requests for information based on the principles set out above. Approved requests will also be reviewed periodically. From December 2004, all requests for information, which NHS foundation trusts do not have a legal or statutory obligation to complete, will require Monitor approval through this process before becoming mandatory. They will then be added to Schedule 6 of the NHS foundation trusts’ Terms of Authorisation.
